*Completed Clinical Program Application from Contract Health Services II, LLC

*Valid Photo ID (Passport, Drivers License or State ID)

*Resume

​

*USMLE Score Results – If taken

​

*Letter of Good Standing from School or Affiliated Medical Education program

​

*PPD Within 6 months

​

*Physical Exam from Primary Physician 6 months to one year

​

*Proof of Immunization to Hepatitis B

​

*Proof of Immunization to Measles Mumps and Rubella

​

*Proof of Malpractice insurance coverage. (Purchased from Contract Health Services II, LLC.)

​

*Criminal Background Check (online for JPH and Chicago Fingerprint office in Chicago)

​

*Flu shot

*OSHA, HIPAA AND BASIC LIFE SUPPORT FOR HEALTH CARE PROVIDERS

​

Note: Though we prefer that OSHA/HIPAA and Basic Life Support to be done at JPH, we have an American Heart Association Instructor which they run these courses on monthly bases. All the above can be done at the hospital on day one upon arrival, you just have to inform us and we will be more than happy to assist.
